Why do you need a tummy tuck?

Abdominoplasty also called a tummy tuck is a procedure which the excess skin and fat tissue in the middle and lower abdominal region are removed. The procedure ensures the abdomen is tighter, smoother and firmer. The abdomen is an aesthetically considerable body region, directly reflected the overall appearance of the person.

The deformities in the abdominal region commonly occur are:

  • Stretch marks on the skin after pregnancy
  • Fat accumulation and sagging
  • Bloating due to the loosening of the abdominal muscles

Abdominoplasty is not a procedure to solve the obesity problem. If you are obese, since the abdominal skin will loosen and descent after weight loss, the surgery would be better after weight loss. The risk of complication of belly surgery also reduces if you lose weight before planning the abdominoplasty.

Following the obesity treatment, when you reach your targeted weight, a tummy tuck corrects your body contours. Even circumferential abdominoplasty may be required.

The good candidates for tummy tuck are:

  • Men and women who have not a serious health problem
  • Having complaints of:
    • Skin loosening and wrinkles in the abdominal area
    • Excessive fatty tissue that cannot be lost despite sport, diet and exercise.

Most of the abdominoplasty candidates who visit me are women who experienced pregnancy in the past. It is also feasible for the women with abdominal sagging due to the massive weight alteration even if they were never pregnant.

Belly surgery is considerable for the men who have saggy abdominal skin.

Some women complain of stretch marks on the abdominal skin. They are in search of getting rid of these marks without belly surgery. However, there is no non-surgical method to vanish the stretch marks completely.

During the abdominoplasty, I remove the skin and underlying fat between the pubic border and a few centimeters above the belly button elliptically. Thus, the stretch marks located on the removed skin also goes away together with the skin.

Ideal candidates for abdominoplasty in terms of patient safety:

  • Clean medical history and to be free of systemic diseases such as heart disease
  • People with well medical conditions and general health status
  • People who are not plump and obese
  • Patients who underwent an unsatisfied tummy tuck before
  • Women who had elapsed at least one year from pregnancy
  • Non-smokers
  • People who did not undergo any major abdominal surgery
  • People who consent the surgical scar and have a realistic expectation

What will I aim in tummy tuck?

My objective is to make you happy by achieving a tight abdomen and proportionate body contour with minimum surgical mark. So, removal of redundant skin and fat will improve your body image and restore your self-confidence.

Some patients seek a solution as they experienced a failed tummy tuck before. Poor scarring due to slipshod suturing of surgical incisions, inadequate tightening, and belly button deformities are the most common complaint of the patients consulting me. Sometimes, their corrective surgery challenges are even more complex than the primary surgery. Therefore your first tummy tuck is crucial. The goal is to achieve the best outcome in a single shot.

Surgical techniques of tummy tuck

There are two techniques for abdominoplasty: full tummy tuck and mini tummy tuck. I determine the technique which will provide best for you as evaluating the severity of the problem.

Mini abdominoplasty is a favorable option if:

  • Loosening and separation of the abdominal muscles (diastasis recti) are not present
  • Skin is elastic and tight above the level of the belly button and looks fine
  • There is skin loosening below the belly button only

Briefly, if the problem is mild and below the belly button, a mini tummy tuck provides a satisfying result.

If similar problems are present above the belly button, a full tummy tuck is needed.

In other words:

  1. Place your hand on 1 cm above the belly button
  2. Place the other hand on the lower border of the abdomen
  3. Pinch the skin between your hands

If your hands can connect when pushing the upper and lower abdomen together, that means you can have a full abdominoplasty. This simple test shows that the necessity of full abdominoplasty in most of the patients.

If you undergo a mini abdominoplasty despite you needing full abdominoplasty, the gain and result would be unsatisfying. Namely, mini abdominoplasty provides a minimal results for the patients who need full abdominoplasty. (Ref. Abdominoplasty: classic principles and technique)

How will I perform full tummy tuck?

I remove the skin and fat, which are redundant between a few centimeters above the belly button and the lower abdominal border in full abdominoplasty.

I get free the belly button from the skin and fat tissue as cutting the inner edge of the belly button circumferentially. However I leave the belly button in its place intact. I divide the skin and fat tissue from the abdominal muscles towards the lower border of the ribs. Thus, the muscles forming the abdominal wall appear. I stitch these muscles to each other by approximating on the midline. Tightening the muscles is an essential part of a tummy tuck.

This maneuver makes them tight in position as if they were in their youth. It also dramatically improves the bloating appearance of the abdomen, also narrows the belly. After the abdominal muscles restoration, I pull down the divided skin and fat tissue as tight as can be, and take out the redundant part of the skin and fat.

Finally I stitch upper and lower edges each other. Meanwhile, I create a new hole on the center of the belly. I stitch the belly button which I left intact by placing it in the new hole. At the end, I place a tube to drain the blood and excess fluid that may collect beneath the skin.

I always design the surgical mark of the tummy tuck where a bikini can easily hide it.

How will I perform extended tummy tuck?

In some cases, the problem may not be limited on the frontal part of the abdomen. An extended tummy tuck is necessary to remove entire fat and skin excess. Especially in overweight patients, the side edges of the skin to be removed may extend to the back of the waist. In such cases, I also remove excessive tissues on the back side of the waist.

How will I perform mini tummy tuck?

In mini abdominoplasty, I remove the excess skin and fat all below the belly button level by cutting elliptically. Loosening and separation of the abdominal muscles on the lower part may occur. In such cases, I also stitch the muscles together on the midline for tightening. After the redundant tissue removal, I stitch the upper and lower edge of the skin together.

In mini abdominoplasty procedure, the belly button is left untouched. Since the surgery is in a limited area, the effect of the procedure is also limited to the lower part of the abdomen. In selected cases, I can also restore the abdominal muscles above the belly button without dividing belly button during mini abdominoplasty.

My personal approach

Tummy tuck requires general anesthesia. The procedure usually takes 2,5-3 hours. However extended tummy tuck may last longer. You will be able to leave the hospital 1-2 days after the procedure.

Tightening the abdominal muscles is already one of the essentials of the tummy tuck.

Apart from muscle tightening, I consider the particular three points in abdominoplasty:

  • I design the incision so a bikini can cover the surgical mark.
  • I create a slightly wide dimple around the belly button to achieve it’s natural look.
  • Normally, abdominal muscles look like a 10 cm in width column on the both sides of the belly button. I create a column-shaped muscle effect with the fat layer underneath the skin, which provides a full natural look.

How is the recovery period after full abdominoplasty?

You will be able to mobilize with the assistance of an accompanying person 3-4 hours after the procedure. The next day, you will get better to mobilize without someone’s assistance.

Although the tummy tuck will be under general anesthesia, I will also inject a local anesthetic to the surgical area. Thanks to long-lasting local anesthetic I will use during the procedure, you will not feel any pain on the first day when the actual pain is mostly felt.

You might hear that a tummy tuck is a too painful procedure. However, astonishingly your pain will be much less than you imagine. Moderate pain that you will feel on the following days can easily be controlled by pain medication.

You will be able to take a shower 3-4 days following the surgery.

However do not forget that you should avoid exhausting movements, coughing, and heavy lifting in first week. 4-5 days later, you will be able to travel.

If you feel as good and confident enough, you can resume to work following 7-10 days of rest. You need 3-4 weeks to begin sportive and physical activities gradually after tummy tuck.

How is the recovery time after mini abdominoplasty?

A mini tummy tuck does not involve as large an area as full abdominoplasty. So the surgery will take 1-1,5 hours. The recovery period is also quicker and shorter. You will be able to resume casual life 2-3 days after the procedure.

Almost all my patients ask me when they will be able to begin daily activities. No need to ask me. When you attempt to do something, just listen to your body. Your body will already say “Go on” or “Stop”. Then you can limit your activities yourself accordingly.

You will also be able to swim after 10-15 days as it’s enough for the wound to heal following the surgery.

How will you feel yourself after the tummy tuck surgery?

Diagram shows, how you will feel yourself emotionally during the recovery period after tummy tuck. The effect of the surgery may impair your emotional status. However, as time goes on, you will recover and get rid of surgical effects; and eventually, you will gain your confidence back.

How will your body look after having the surgery?

In a tummy tuck, I remove not only excess tissue, but also create a new belly button that looks original. In this manner, the skin surface around the belly button does not look flat. That is, I also achieve the dimpling of its natural form.

Therefore nobody can notice that you had an abdominoplasty even if you wear a bikini. I always stitch the skin with absorbable and hidden sutures, you will not feel stitch removal stress.

Immediately after the surgery, a dramatic improvement in the appearance of the abdominal region occurs. The improvement you will experience significantly increases your life quality. Abdominoplasty not only ensures you get rid of excess and sagging tissues, but also increases your confidence in yourself.

What are the risks and complications of the tummy tuck?

The complication rates of tummy tuck are the compilation of medical literature data. The rates may vary depending on the patient’s medical conditions. That does not mean you will experience all those complications.

Seroma is the most common complication, which is fluid collection inside the wound. It is a temporary problem, not a scary serious complication. Usually, extensive dissection leads to a fluid collection inside. Fluid collection stops  on its own in 1-2 weeks after the surgery.

Infection and bleeding are general complications of all surgical procedures. Hematoma (blood collection inside the wound) is not usual when the plastic surgeon carries out the procedure thoroughly. Infection is preventable as taking post-operative antibiotic medication.

The most important issue regarding the risk factor of tummy tuck is smoking. It is considerable that how long you have been smoking and how many pieces you consume per day. Smoking causes delayed wound healing and even detachment of the wound. It is better to stop smoking prior to the abdominoplasty to prevent such problems.

Diabetes and obesity are also risk factors upon wound healing quality. In case of any complication that may discomfort you and prolong the recovery period, this can be managed and treated as well.

Although it is not considered as a risk, another issue is that the surgical scar remains noticeable in some patients. A fresh surgical scar looks red and fairly noticeable in the first weeks. The color of the scar fades in a few months, and turns into the normal color and becomes less noticeable. (Ref.
