Why do you need face change?

Face change surgery is one of the rarest and most extreme procedures in the world of plastic surgery. Only a handful of plastic surgeons get the chance to perform this type of surgery in their careers, often depending on what the patient desires. Fortunately, I’ve had the opportunity to experience this intense procedure multiple times.

The reasons behind a person’s desire to change their face can vary greatly:

• Anybody may wish to look different
• Males may wish to look like a female (Facial feminization surgery)
• Females may wish to look like a male (Facial masculinization surgery)

What is face transformation?

The film industry has led many to believe that you can completely change your face into someone else’s. This has made some individuals, unhappy with their own facial features, aspire to look like their favorite celebrities.

I remember a man who came to see me years ago, asking if I could transform his face to look like Johnny Depp’s. Naturally, I had to turn him down because his expectations were simply unrealistic. Such transformations are the stuff of science fiction.

I’m just a surgeon, not a magician. That said, I have helped some patients who wanted to change their appearance without aiming for a specific celebrity look. The results are usually variations of their original features. However, the more procedures you undergo, the more your face can change.

It also depends on how much freedom you give me in altering your appearance. If you allow me to make unrestricted changes, I can create a face that looks entirely different.

There are many applications and software that can simulate facial reshaping in 3D, allowing you to see potential changes before surgery. But transforming a face on a computer and doing it in real life are two very different things.

While digital media can provide a glimpse of how your face might change, no surgeon can guarantee that the final result will be exactly what you see on screen.

Psychologic aspect of face change surgery

Before considering a face reshaping procedure, it’s essential to take a moment and reflect on your expectations and the reasons behind wanting such a significant and potentially irreversible surgery. Your mental readiness is key here. Ultimately, how satisfied you feel with the results will hinge on what you expect.

Based on your expectations, I categorize candidates into two groups:

1. Those aiming for a specific face shape: As I’ve mentioned, I’m not a magician who can create an exact replica of the image you have in your mind. If your expectations are rigid and well-defined, you might end up feeling disappointed. That’s why I won’t proceed with your surgery in this case.
2. Those who are open to various outcomes: If your primary goal is simply to change your face in some way and you’re flexible about the results, you’re likely to be pleased. In this scenario, I’ll work with you to design a look that aligns with your wishes.

The units and factors that define the face shape and appearance are basically:

• Facial bone formation
• Facial muscle formation
• Fat tissue thickness and distribution
• Skin characteristics
• Hair characteristics

The endless variations in how those units can be formed allow for unique face shapes among individuals. The surgical parameters that can be altered for face reshaping include:

Bone shape variations

The bones of the face which influence face shape consist of lower jaw (mandible), upper jaw (maxilla), cheek bones (zygoma), nose bone (nasal bone), and forehead bone (frontalis).

Lower jaw (Mandible)

• Width of the lower jaw arch: Reduction and widening
• Sharpness of the rear angle of lower jaw: Smoothening and rounding
• Distance between teeth and lower margine of jaw: Increasing or decreasing
• Prominency of the chin (front part of lower jaw): Reduction or augmentation
• The position of lower jaw relative to upper jaw: Advancing forward or pushing backward

Upper jaw (Maxilla)

• Prominency of upper jaw: Advancing forward or pushing backward
• Distance between upper teeth and nose: Increasing or decreasing

Cheek bone (Zygoma)

• Bone prominency: Reduction or augmentation
• Bone arch width: Widening or narrowing

Nose bone (Nasal bone)

• Size: Reduction or augmentation
• Width: Widening or narrowing
• Tip width: Narrowing or widening
• Tip angulation: Rotating upward or downward

Forehead bone

• Forehead bone prominency: Augmentation is feasible only. The forehead bone is one of the bones that protects the brain. Reduction of the forehead bone makes the bone thinner and leads it to become more fragile against trauma.

Facial muscle formation

• Facial muscles influence face appearance visually and also have an importance functionally. Muscles are the sole dynamic component of the face. Muscles ensure all mimic movement including opening and closing of the eyes and mouth, smiling, speaking and many more… Since changing the muscle formation also affects their movement functions, only minor changes are feasible in order to preserve their functions.

Fat tissue thickness and distribution

• The volume of the fat tissue influences facial appearance. Large volume of fat leads a person to look chubby, in contrary, little volume of fat leads person to look skinny. Both volume reduction for chubby people and volume enhancement for skinny people are feasible. While liposuction and / or buccal fat removal ensures fat reduction, fat transfer ensures fat volume enhancement.
• Lip augmentation: Fat injection or filler injection makes your lips fuller.

Skin characteristics

• Skin tightness: Skin tightening on your cheeks and eyelids is feasible which leads you to look younger
• Eye shape: Converting an Asian shaped eye to European shaped eye, in contrary converting an European shaped eye to Asian shaped eye is feasible. Also elevating the outer corner of the eye is feasible which leads to significant difference upon the facial expression.
• Lip reduction: Removing a particular width of the skin on the reddish part of the lip makes your lips look thinner.
• Nevus (mole) transfer: Almost everybody has a nevus on various part of their body. It is feasible to transfer any nevus from any part of the body onto the face.

Hair characteristics

• Hair density: Hair implantation makes your hair to look more dense. Not only for head hair, but also it is feasible to do for eyebrow, beard and moustache. In contrary, laser hair removal provides an option to reduce your hair.
• Eyebrow width: Widening or narrowing

Sudden radical changes on your face image may likely impair your emotional status, and be hard to adopt your new face. What are the distinctive characteristics between feminine and masculine faces?

Forehead characteristics

• The distance between the eyebrows and hair line is wider in males than females. Therefore male’s forehead looks wider than the female’s forehead.
• Bone ridge above the eye is more prominent in males than females.

Eyebrow characteristics

• Female eyebrows look more arched, whereas male eyebrows look straighter.
• The distance between the eye and eyebrow is wider in females than the males. In other words, female eyebrow looks higher than the male eyebrow.

Eye shape characteristics

• Since the ridge of the frontal bone is more prominent on the eyebrow level in males, male eyes look deeper set than female eyes.
• When eyelids are open, the distance between the upper and lower eyelid margin is wider in females than males. In other words, female eyelids appear more open, whereas male eyelids appear more closed relatively.

Nose shape characteristics

• Females noses appear smaller than male nose.
• The angle between the nose and upper lip is wider in females than males. In other words, the tip of the nose is more rotated upward in females than that of the males.
• The width of the female nose is narrower than that of the male.

Cheek characteristics

• Females have more prominent cheek bones than that of the males. Male cheek bones are flatter.
• The fat layer on the cheek is thicker in females than that of males. Therefore cheeks look fuller in females.

Lip characteristics

• The distance between the upper lip and nose is wider in males that that of the females.
• Female lips appear fuller, whereas male lips appear thinner

Jaw characteristics

• The male has a wider and stronger jaw with a sharper angle. In contrary the female jaw appears weaker, narrower with a smoother and rounded angle.
