Chin surgery simplified
Why do you need chin augmentation or reduction?
The structure, shape, and size of the facial bones may vary from person to person, which directly reflects the appearance of the face. Therefore the size and shape of the bones are the most influential factors that affect the facial expression of people.
Chin augmentation and reduction can change the feature directly. Thus the deformities of facial bones can cause an aesthetic issue in facial expression.
Furthermore the deformities of the jaw bone, may also cause functional chewing problems. In such cases, maxillofacial surgery which is a subdivision of plastic surgery helps to correct such problems.
Chin augmentation or reduction can also be combined with rhinoplasty in cases where the facial profile view needs improvement. Such combined procedures are useful for the correction of an underdeveloped chin accompanied by a beak-like nose, called “bird face deformity”.

Chin Augmentation Surgery
There are two techniques for chin augmentation:
Chin Augmentation With Osteotomy
This technique of chin augmentation is a relatively more complicated and exhausting technique. I cut the mucosa 3-4 cm inner aspect of the lower lip’s base and elevate soft tissue off the chin bone. A powered saw cuts a piece of bone from the chin. I relocate the bone piece forward to increase the projection of the chin, then fix it with screws.
The advantage of chin augmentation with osteotomy technique is that of the scar which remains inside the mouth. There will be no visible scar outside.
However chin augmentation with osteotomy has disadvantages. Since it is more complicated and more exhausting, many surgeons do not prefer this. Surgical time takes longer than chin implant techniques.
Chin Augmentation With Implants (Chin Implant Surgery)

Chin implant placement is the most common and relatively simpler technique for chin augmentation. The surgeon accesses the chin bone either inside the mouth or below the chin.
1- Intraoral chin augmentation
I cut the mucosa 3-4 cm inner aspect of the lower lip’s base. It is simple to elevate soft tissue off the chin bone. Then I place the chin implant in front of the chin bone and fix it using screws. The advantage of this technique is to not leave a visible surgical scar, because the skin cut is performed inside the mouth. However the disadvantage of the technique is a slightly high risk of infection, because inside the mouth has very rich bacterial flora.
2- Submental chin augmentation
I cut 3-4 cm of the skin below the jaw and create a pocket over the bone just beneath the skin. After the chin implant placement and fixation with the screws, the wound is sutured. This technique of chin augmentation is easier than intraoral approach. Another advantage is less infection risk. However it leaves a 3-4 cm surgical mark below the chin.
Chin Reduction Surgery
Chin reduction is useful to reduce an oversized jaw bone in order to achieve a natural balance of the face proportion. Suitable candidates should have normal occlusion, namely, the upper and lower teeth should fit each other.
Differentiating the mandibular prognathism is critical. In mandibular prognathism, the lower jaw is positioned more forward relatively to the upper jaw, so the lower teeth and upper teeth do not fit each other and causes a gap between them. In such cases, sagittal split osteotomy techniques are required to push back the lower jaw rather than chin reduction surgery.
In a chin reduction procedure, I incise the mucosa 3-4 cm inner aspect of the lower lip’s base and elevate the soft tissue off the lower jaw. After I expose the bone completely, I mark the cutting line of the bony excess. An oscillating saw is a very useful device and it facilitates to cut the bone. After bone removal, I refine the sharp edges of the bone as rasping to get a smoother bony surface. In the end, I close the surgical cut with absorbable sutures.
How is the recovery period after chin surgery?
Patient satisfaction rate after the chin surgery
You may leave the hospital on the same day whether you had chin augmentation or chin reduction. I will prescribe your medicines which includes antibiotics and pain killers. The surgery, either chin augmentation or chin reduction is not so painful, however you should take pain killers to relieve the pain.
If your surgical wound is inside the mouth, you should also use mouth wash for 4-5 days. During this period I recommend you eat soft foods.
You should wear a jaw garment for 1 week following chin surgery.
Concomitant swelling will resolve within 7-10 days. You will see the actual result immediately after the surgery.
However settling down of the soft tissues overlying the bone takes longer time after chin reduction surgery. Therefore the result appears 1-2 months later.
Diagram shows, how you will feel yourself emotionally during the recovery period of chin surgery. The effect of the surgery may impair your emotional status. However, as time goes on, you will recover and get rid of surgical effects; and eventually, you will gain your confidence back.
