Why do you need dimple creation?
Dimples have long been thought of as a simple inherited trait — the old belief was that two dimpled parents would always have a dimpled child — but the genetics are in fact more complex than that, and even two parents with dimples may have a child without them. A natural cheek dimple comes from a small developmental quirk in the cheek muscle, and while most dimples last a lifetime, some fade as the facial muscles gradually weaken with age, which is part of why dimples are so associated with youthfulness and charm.

What kinds of dimples can be created?
Not all dimples are the same, and I create three different kinds, each in the place and manner that suits it.
Cheek dimples
Cheek dimples are the most common request, and they sit a few centimeters in from the corner of the mouth. They can be made on one side or both — natural cheek dimples appear on both sides in only about one in three people — so we decide together whether one or two best suits your smile.
Chin dimples
The chin dimple, set in the middle of the chin, is the second most common type. It gives definition to the chin and, for the right face, a distinctive, characterful look.
Back (Venus) dimples
Back dimples — often called Venus dimples — are the least common and sit just above each side of the lower back, above the buttocks. Because of where they are and how they form, they call for a quite different technique from facial dimples, which I describe below.

How does Dr. Baycin perform dimple creation surgery?
Dimple creation is a small, quick procedure, and because the treated area is tiny, local anesthesia is all that is needed; each dimple takes only about thirty to forty minutes. For a cheek or chin dimple, I make a small incision, about one centimeter, inside the mouth — so there is never a scar on the outside — and through it I remove a little fat to create the hollow, then place an internal, dissolving stitch that gently tethers the skin inward to form the dimple. That internal stitch is what makes the result last, and it dissolves on its own within about seven to ten days.
A back (Venus) dimple is a different matter, because of where it sits: here I use the Vaser liposuction technique to gently reduce the fat over a small round area of about two to four centimeters, removing just three to four millimeters of fat — enough to create that soft, hollowed dimple. Because both cheek/chin dimples and my buccal fat removal are done from inside the mouth, the two can be combined comfortably when it suits a patient’s goals. The published evidence puts numbers on why placement matters so much: in a single-clinic review of 2,048 dimple creations, medical complications were very rare — infection led to suture removal in five patients — but 7.7% of patients returned for a second operation, almost always for an aesthetic reason: undercorrection, or the dimple fading away.

Dimple creation asks almost nothing of the body and a little of the nerves. You go back to your routine the same day; there may be a mild ache for a day or two, easily managed with simple painkillers, and because the incision is inside the mouth you keep to soft foods and an antiseptic rinse after each meal for four to five days. The internal stitch dissolves on its own between days seven and ten, and by then the small bond between skin and muscle that holds the dimple has formed. The mood runs a different course. On day one the dimple looks too deep and too sharp, because the stitch is at full tension, and most people dislike the first glimpse in the mirror. Over the first week it softens as the stitch loosens, shows mostly when you smile, and by the second week it simply looks as though it was always there. The pace varies with personality; the shape rarely does.
How will your dimple look afterward?
Right after your dimple surgery, you might notice that your dimple looks quite pronounced due to the tension from the internal stitch. But don’t worry. As the stitch loosens and eventually dissolves, your dimple will start to look softer and more natural.

There are a few things to understand. First, a dimple can flatten and fade over time, and how well it lasts depends genuinely on the surgeon’s skill and the technique used. Second, because the cheek dimple is created close to the buccal nerve and the surrounding muscles, there is a small risk of nerve or muscle involvement, which is why precise, anatomy-aware placement is so important. And third — the most important point of all — the reported dissatisfaction rate for dimple surgery is higher than for many other cosmetic procedures, almost always because a dimple can end up looking unnatural, and some patients then ask for it to be reversed.
