I always opt for the endoscopic forehead lift technique. This procedure typically requires local anesthesia, often combined with deep sedation or general anesthesia, and it usually takes about 1.5 hours to complete.
During the endoscopic forehead lift, I make a small incision, about 1.5 to 2 cm long, just beyond the hairline. I create incisions on both temples and one on each side of the forehead. Then, I insert an endoscopic camera, which is roughly 0.5 cm in diameter, through these openings. At the same time, I introduce specially designed instruments for endoscopic surgery through additional small cuts.
Using the camera’s view, I gently lift the skin away from the bone down to the eyebrow level. The camera makes it quite straightforward to elevate the skin. Once the skin is lifted, I carefully weaken the muscles that create forehead lines by making fine scores, which helps to release the skin. Finally, I pull the skin upward and secure it in a tighter position.
There are several fixation methods available. Some surgeons prefer to drill a small hole in the bone and use stitches to secure the forehead skin through that hole. Others might choose to use 3-4 mm screws for fixation, while some opt for specially designed hangers attached to the bone with screws.
Regardless of the method chosen, all of them effectively hold the brow skin tighter against the bone. In terms of the surgical outcome, it doesn’t really matter which technique the surgeon selects. To finish, I close the skin incisions with absorbable stitches.
